Counseling Partners of Los Angeles Informed Consent Forms

Counseling Partners of Los Angeles offers personal counseling to students at Cathedral High School.  In order to see a counselor, students need to submit an Informed Content form signed by the student and parent.  Consent forms are posted below.  Please print the appropriate form and return to the Main office, attendance office, or grade level counselor.  If needed, students and parents may pick up a printed copy in the main office or from grade level counselors.

In order to be seen by a CPLA counselor, students and/or parents need to submit a form.  Forms can be found in the Counseling Office.  Copies are archived below.  Forms need to be re-submitted each year.
